The medical record is a private, mandatory, and confidential document that records health conditions, medical procedures, and other procedures performed by the healthcare team. This document may only be accessed by third parties with the patient's prior authorization or in the cases provided for by Law 23 of 1981.<\/p>

A copy of the medical record may be requested by the patient themselves; by a legal representative if the patient is a minor; by a person directly authorized by the patient; and, if the patient is incapacitated or deceased, by providing a document certifying this status.<\/p>

Attached to the completed form must be the patient's document and\/or documents of the family member or third party making the request.<\/p>

This procedure can be carried out in the same way, sending all the requested documents scanned to the email\u00a0servicioalcliente@virgiliogalvis.com.co<\/strong><\/a>\u00a0and your request will be answered within 48 hours.<\/p>
